Sebbene il metabolismo del colesterolo sia un percorso comune per lo sviluppo di farmaci antitumorali, non esistono bersagli e farmaci specifici per uso clinico. Qui, sulla base del nostro precedente studio sulla sterolo O-aciltransferasi 1 (SOAT1) nel carcinoma epatocelluare, abbiamo cercato di selezionare un farmaco mirato efficace per il trattamento preciso del carcinoma epatocelluare e, dal punto di vista del metabolismo del colesterolo, chiarire la relazione tra regolazione del colesterolo e tumorigenesi e sviluppo.
In questo studio, abbiamo sviluppato una tecnologia di screening di affinità integrata per lo screening virtuale per lo screening dei farmaci con proteine bersaglio. Per la verifica dell’attività del farmaco sono stati utilizzati una serie di esperimenti in vitro e in vivo. L'analisi multi-omica e l'analisi della citometria a flusso sono state utilizzate per esplorare i meccanismi antitumorali. L'analisi comparativa del proteoma e del trascrittoma combinata con le informazioni di follow-up sulla sopravvivenza dei pazienti rivela il potenziale clinico terapeutico dei farmaci selezionati.
Abbiamo esaminato tre composti, nilotinib, ABT-737 ed evacetrapib, che mostravano un legame ottimale con SOAT1. In particolare, nilotinib ha mostrato un’elevata affinità per la proteina SOAT1 e ha inibito significativamente l’attività tumorale sia in vitro che in vivo. L'analisi multi-omica e l'analisi della citometria a flusso hanno indicato che i composti mirati a SOAT1 hanno riprogrammato il metabolismo del colesterolo nei tumori e hanno potenziato le cellule T CD8+ e i neutrofili per sopprimere la crescita del tumore.
Nel loro insieme, abbiamo segnalato diversi ligandi SOAT1 ad alta affinità e dimostrato il loro potenziale clinico nella terapia di precisione del cancro al fegato e rivelato anche il potenziale meccanismo antitumorale dei composti mirati a SOAT1.

Studi precedenti hanno scoperto che la riprogrammazione del metabolismo del colesterolo tumorale avviene durante la tumorigenesi e lo sviluppo [1, 2]. Le caratteristiche del metabolismo del colesterolo dei tumori includono sia la sovraregolazione della sintesi e dell’assorbimento del colesterolo sia l’accumulo di vari derivati del colesterolo, come gli esteri del colesterolo e il colesterolo ossidato [3]. Tuttavia, la riprogrammazione del metabolismo del colesterolo nei tumori coinvolge molti processi, tra cui sintesi, assorbimento, esterificazione, efflusso e conversione. Pertanto, chiarire il ruolo di questi processi nella tumorigenesi e nello sviluppo di potenziali farmaci terapeutici è stato impegnativo.
Molti recenti studi clinici e preclinici hanno dimostrato che mirare al metabolismo del colesterolo nelle cellule tumorali e nelle cellule immunitarie è un modo ovvio per trattare i tumori [4]. Gli obiettivi esistenti includono enzimi chiave nella sintesi e nelle vie di trasporto del colesterolo, nonché fattori chiave di trascrizione coinvolti nel metabolismo del colesterolo. Ad esempio, le statine esercitano effetti antitumorali inibendo l’attività dell’enzima chiave limitante la velocità 3-idrossi-3-metilglutaril-coenzima A reduttasi (HMGCR) nella via di segnalazione della sintesi del colesterolo [5], e l’itraconazolo si lega direttamente allo sterolo- dominio sensibile del trasportatore intracellulare del colesterolo NPC 1 (NPC1), riducendo così la crescita del tumore e l'angiogenesi [6]. Inoltre, LXR-623, GW3965, altri inibitori di piccole molecole contro il recettore X del fegato (LXR) e altri farmaci a piccole molecole sono attualmente oggetto di studio come farmaci antitumorali [7].
Si prevede che la scoperta di nuovi bersagli farmacologici correlati al colesterolo accelererà lo sviluppo di farmaci mirati all’omeostasi intracellulare del colesterolo per la terapia dei tumori. La proteina SOAT1 può convertire il colesterolo intracellulare in eccesso in estere del colesterolo e immagazzinarlo in goccioline lipidiche. L'abbattimento o l'inibizione di SOAT1 può inibire una varietà di tumori regolando l'omeostasi del colesterolo nelle cellule tumorali [8, 9] e lo stato immunitario del microambiente tumorale [10, 11]. Abbiamo precedentemente scoperto che SOAT1 può essere utilizzato per stratificare i pazienti con HCC con prognosi sfavorevole e come bersaglio farmacologico per il trattamento di pazienti con questo sottotipo. È stato dimostrato che gli inibitori che prendono di mira SOAT1 esercitano effetti antitumorali ottimali sia in vitro che in vivo. Tuttavia, ci sono solo pochi inibitori SOAT1 conosciuti – sono stati segnalati solo avasimibe, nevanimibe e CI-976 – il che limita notevolmente le prospettive di sviluppo del farmaco.
